Job summary

Talin Home Healthcare LLC is seeking compassionate Home Health Nurses to provide skilled nursing care in patients' homes across the Northern Virginia area. You will assess patients, create care plans, administer meds, and coordinate with physicians to deliver high-quality, personalized care.

Candidates must hold an active Virginia RN license, have strong clinical skills, and be able to travel. We offer flexible scheduling with a minimum commitment of two days per week, including either Monday or

Qualifications

• Active RN license in Virginia (or state equivalent)
• Oasis SOC experience preferred
• Previous home health, med-surg, or community nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver’s license for travel throughout NOVA
• Ability to work independently and manage a flexible patient caseload
• Commit to working a minimum of 2 days a week, 1 day must be Monday or Tuesday

Responsibilities

• Provide skilled nursing care to patients in their homes across the Northern Virginia area
• Conduct comprehensive patient assessments and develop individualized care plans
• Administer medications, wound care, IV therapy, and other clinical treatments as ordered
• Monitor and document patient progress, vital signs, and changes in condition
• Educate patients and family members on disease management, medications, and home care techniques
• Collaborate with physicians, therapists, and care team members to ensure coordinated patient care
• Maintain accurate and timely clinical documentation in compliance with regulatory standards
