Qualifications

• Current nursing license in the practicing state.
• Valid driver’s license and reliable transportation.
• Current CPR certification.
• Two years of LPN/LVN experience in a clinical setting.

Responsibilities

• Provide skilled nursing care to patients in their homes under RN direction.
• Implement individualized care plans with patient, family and healthcare team.
• Educate patients and families on disease management and self-care.
• Maintain accurate records of patient care, visits, interventions and outcomes.
• Coordinate with patients, physicians and care team and participate in discharge planning.
