Job summary

APEX Rehab Group seeks a motivated Home Care Physical Therapist to deliver in-home evaluations and treatments in Brooklyn, NY. This role lets you build and manage your own caseload with autonomy and a flexible schedule.

You will develop individualized care plans, document progress, and collaborate with patients and the care team while complying with regulations and standards. A NY PT license is required, with travel within the service area expected.

Responsibilities

• Communicate effectively with patients, families, and healthcare team members.
