Job summary

Easterseals is hiring a Community Inclusion Specialist to work with individuals with disabilities, helping them achieve vocational goals and improve life skills. This full-time role supports diverse programs within Employment and Community Services and may involve field-based duties.

The specialist provides hands-on training, implements plans, and communicates with interdisciplinary teams to empower independence and successful community living.

Qualifications

• This position requires 21+ years of age and a valid driving record.
• CPR and First Aid certification, PMT, and DDS/state trainings within required timeframes.
• Eligible to obtain PSL based on program needs.
• Strong communication and problem-solving skills.

Responsibilities

• Works directly with individuals in employment settings and programs to meet goals.
• Implements individualized treatment plan goals and reporting within interdisciplinary teams.
• Provides on-site employment and life skills training and community liaison.
• Assists individuals in making personal choices and accessing community activities.
• Develops community relationships to increase independence.
• Provides transportation as assigned.
• Assists with personal care per program requirements.
• Completes required documentation ( Therap, binders, emails ).
• Attends required trainings and meetings; performs other duties as needed.
