Job summary

VitalCore Health Strategies seeks a Full-Time Physical Therapist to evaluate and treat incarcerated patients as part of an interdisciplinary team. The PT will primarily work at our Lancaster, MA site but may serve other MA facilities, including Gardner, Shirley, Framingham, Concord, Norfolk, Bridgewater, and Roslindale.

Qualifications

• Must possess a current, unrestricted license as a Physical Therapist in the practicing state.
• Must be fluent in English language, including reading, writing, speaking, and understanding.
• Must be a supportive team member, who contributes to and demonstrates teamwork and team concept.
• Able to make independent decisions when circumstances warrant such action.
• Able to deal tactfully with personnel, family members, visitors, government agencies/personnel, and the general public.
• Possess leadership and supervisory ability and be able to work harmoniously with and supervise other personnel.
• Must not pose a direct threat to the health or safety of other individuals in the workplace.

Responsibilities

• Will be required to work within a correctional facility and travel to multiple facilities.
• Documents encounters in patient's medical record as assigned.
• Ensures Medical Record documentation is in SOAP format, problem oriented and corresponds to the therapeutic order.
• Ensures all documentation is timed, legible, and signed.
• Ensures patient observation for therapy evaluations, formulates treatment plans, and provides treatment.
• Analyzes treatment plans utilizing various technique which may include therapeutic exercise, manual therapy, functional training, and mechanical modalities.
• Documents pertinent observations and logical conclusions to validate need for treatment.
