Job summary

Evolve Therapy Services, LLC in Huntingdon, PA seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility. The role supports planning and directing SLP services to ensure high-quality resident care under CMS guidelines.

Full Time or PRN opportunity. Requires a Master's in Speech Language Pathology, CCC, and an active license in the relevant state(s). Competitive pay and growth opportunities are available based on status.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Active SLP license in the state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop a treatment plan with target list, short-term and long-term goals.
• Instruct patient, family, and staff to carry over therapy goals in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening to ensure resident needs are met timely and with clinical excellence.

Education

Certificate of Clinical Competency

Active SLP license in state(s) of practice
