Job summary

Evolve Therapy Services in Wilson, NC seeks a licensed Speech-Language Pathologist for an in-house Skilled Nursing Facility. The role offers Full Time, Part Time, or PRN positions, with responsibilities spanning evaluation, treatment planning, documentation, and collaboration with residents, families, and the care team.

Requirements include a Master’s degree in Speech-Language Pathology, CCC, and an active SLP license in the state.

Qualifications

• Master’s degree in Speech-Language Pathology
• CCC (Certificate of Clinical Competency)
• Active SLP license in state(s) of practice
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with target list, short-term, and long-term goals.
• Instructs patient, family, and staff in speech-language procedures for carryover of remediation.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols with site program coordination.
• Participate in screening protocols to ensure needs are met timely and with clinical excellence

Education
