Evolve Therapy Services is seeking a part-time Speech Language Pathologist for an in-house Skilled Nursing Facility in Pahokee, FL. The role supports residents with comprehensive speech therapy services on-site.

The successful candidate will plan, implement, and document treatment plans, coordinate FEES/MBS, and ensure CMS-compliant records using NetHealth EMR. Requires SLP Master’s, CCC, and active licensure, plus ongoing CE.

Qualifikationen

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency is required.
• Active SLP license in state(s) of practice is required.
• Ongoing continuing education to maintain licensure is expected.

Aufgaben

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with targets and goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Support screening to identify resident needs with clinical excellence.
