Qualifications include an MA/MS in communication disorders (ASHA-approved), current state licensure, and CFY completion. The role emphasizes comprehensive evaluations, treatment planning, and documentation within a collaborative

Qualifications

• Must hold a current ASHA-certified SLP license or eligible for state licensure.
• Completed CFY as part of ASHA requirements.
• Graduate degree in communication disorders from an accredited program.

Responsibilities

• Provide comprehensive speech therapy evaluations per MD orders.
• Document findings using standardized formats with accuracy.
• Develop and implement treatment plans with goals, frequency and modalities.
• Obtain MD orders for evaluations, treatments, recertifications and discharges.
• Deliver treatments with appropriate modalities and precautions.
• Document progress, discharge plans and MDS-related information timely.
