Job summary

Continuum Rehab Group seeks a dedicated Physical Therapist to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities in Albuquerque. You will deliver patient-centered care, develop rehab programs, and collaborate with a clinical team to improve residents' recovery and quality of life.

Qualifications

• Graduated from an accredited University with a Doctor of Physical Therapy (DPT) and valid state licensure (or in process).
• Experience levels: open to therapists at all experience levels.
• Strong skills in evidence-based physical therapy interventions and patient-centered care.

Responsibilities

• Utilize evidence-based practice to guide clinical interventions.
• Provide rehabilitative and skilled therapy services to patients with physical functioning disorders.
• Supervise Physical Therapy Assistants, students, and support staff as per licensure rules.
• Consult and counsel patients and families about treatment plans and progress.
