Job summary

Creative Solutions In HealthCare is seeking a full-time Physical Therapist (PT) to provide person-centered care that helps residents improve mobility, strength, and overall quality of life.

You will assess patients, interpret evaluations, develop and adjust individualized rehabilitation plans, administer treatments, document thoroughly, and collaborate with the interdisciplinary team.

Qualifications

• Active Texas Physical Therapist license.
• SNF or long-term care experience preferred.
• Knowledge of standard physical therapy practices.
• Experience with Net Health EHR preferred.
• Strong communication and teamwork abilities.

Responsibilities

• Consult with patients to assess physical condition and functional limitations.
• Evaluate and interpret assessments and test results.
• Develop and adjust individualized treatment plans.
• Administer prescribed physical therapy treatments.
• Document thoroughly and timely per policy and regulations.
• Collaborate with interdisciplinary team to ensure quality care.
