Job summary

Avir at Terrell in Terrell, TX, is seeking a Full-Time Physical Therapist to join our in-house rehab team. You will assess patients, develop and implement personalized treatment plans, and collaborate with an interdisciplinary team to achieve positive outcomes for residents in our skilled care setting.

Qualifications

• Active/Valid Texas Physical Therapy license
• In good standing with regulatory agencies and licensing boards
• Experience in SNF/long-term care preferred

Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment and adjust as needed in collaboration with the IDT.
• Provide education to patients, family, and caregivers.
• Collaborate with interdisciplinary team to ensure quality patient care.
• Ensure documentation complies with regulatory requirements and is timely.
