Job summary

Evolve Therapy Services is seeking a Physical Therapist for our partner in Minot, ND. This in-house role supports an SNF with part-time or PRN scheduling, focusing on high-quality resident care and interdisciplinary collaboration.

The PT will assess, plan, and implement treatment programs, document outcomes, and ensure compliance with CMS requirements using NetHealth EMR. Ongoing professional development is encouraged.

Qualifications

• Bachelor degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in applicable state(s).
• Commitment to ongoing education to maintain licensure.

Responsibilities

• Provide PT services to residents per standards and productivity.
• Document and bill PT services per company procedures.
• Evaluate residents and develop treatment plans with goals.
• Screen residents to determine need for intervention.
• Communicate evaluation findings to residents, families, and team.
• Complete CMS-compliant documentation in NetHealth EMR.
