Job summary

The Center at Blue Hills in Stoughton, MA is actively seeking a full-time and PRN Physical Therapist Assistant to join an in-house therapy team focused on Alzheimer’s and dementia care. This SNF with a primarily psych/dementia caseload offers supportive collaboration with therapists and nursing staff.

The role requires graduation from an accredited PTA program, a current MA PTA license, and CPR/BLS certification.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current state PTA license in good standing.
• CPR/BLS certification (required).
• Previous experience in a skilled nursing or subacute rehab setting preferred.
• Strong interpersonal and communication skills.

Responsibilities

• Administer treatment to residents and patients to relieve pain and improve function.
• Ensure treatment is delivered per plan of care.
• Provide clinical support and educate patients, families, and caregivers.
• Monitor patient response to treatment interventions.
• Complete required forms and documentation per regulations.
• Consult with patients about their physical condition and treatment options.
• Develop treatment plans and personalize fitness-oriented programs.
• Document patient care history and ensure compliance with policies.
