Job summary

Evolve Therapy Services in New Haven, CT, is seeking a licensed Occupational Therapist for an in-house Skilled Nursing Facility. This full-time or PRN role focuses on delivering comprehensive OT services to residents.

You will plan, organize, develop and direct therapy, coordinate with an interdisciplinary team, ensure accurate documentation and billing, and promote the highest standard of resident care. Active OT license in the practicing state is required, with ongoing continuing education to

Qualifications

• Degree in Occupational Therapy.
• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screenings of residents to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans; set short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and team members.
• Establish individual resident therapy schedules with team.
• Conduct therapy according to plan and ensure documentation and billing.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence.
