Job summary

Evolve Therapy Services is seeking a licensed Occupational Therapist to provide in-house therapy at a Skilled Nursing Facility in Sutherland, Nebraska. This PRN opportunity focuses on delivering high-quality occupational therapy services, planning interventions, and collaborating with the care team to meet resident goals.

You will screen residents, develop treatment plans, document care, and supervise COTA as needed.

Qualifications

• Must hold an active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy from accredited program.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Screen residents to determine best intervention for each patient.
• Evaluate residents and develop treatment plans with goals.
• Communicate evaluation findings and plans to residents and families.
• Create individual therapy schedules with the care team.
• Deliver therapy per plan and document and bill services.
• Supervise COTA and ensure compliant documentation and reimbursement.
• Perform other duties to effectively treat patients.
