Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Midwest City, OK. This PRN role focuses on delivering high-quality OT services within a collaborative team.

You will plan, organize, develop, and direct Occupational Therapy services in accordance with applicable standards, ensuring the highest quality resident care while maintaining proper documentation and billing. Federal, state, and local guidelines apply.

Qualifications

• Degree in Occupational Therapy.
• Active OT license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of OT services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Ongoing continuing education and professional development to maintain licensure and certification
