Job summary

Evolve Therapy Services, LLC seeks an Occupational Therapist for an in-house Skilled Nursing Facility located in Oil City, PA. The role requires planning, organizing, developing, and directing Occupational Therapy services to ensure high-quality resident care in compliance with standards and regulations.

Resp onsibilities include screening and evaluating residents, creating treatment plans with goals, communicating findings to families and the team, scheduling therapy, documentation and billing,

Qualifications

• Degree in Occupational Therapy is required.
• Active Occupational Therapy license in practicing state(s) is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine appropriate intervention or treatment.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings and treatment plans to residents, families, and interdisciplinary team.
• Establish individual resident therapy schedules with team collaboration.
• Document and bill Occupational Therapy services according to policy.
• Oversee COTA and ensure documentation and treatments meet clinical and reimbursement standards.
