Job summary

Sweetwater Care, a family owned long-term care facility in Northern Colorado, seeks a compassionate Certified Occupational Therapy Assistant to join our midsize facility in Larimer County. We offer flexible scheduling options and are supported by a seasoned clinical team within a low-staffing-ratio environment.

The role involves treating patients under OT supervision, documenting progress, maintaining the therapy department, and participating in care conferences.

Qualifications

• Licensed Occupational Therapy Assistant
• Experience with geriatric therapy and long-term care settings preferred
• Ability to document treatments and progress accurately
• Strong communication with multidisciplinary team

Responsibilities

• Treat patients as directed by the OT and document treatments
• Record treatments and progress notes in medical records
• Assist in maintaining department operations
• Participate in patient care and rehab conferences
• Assist with cleaning and maintenance of treatment area
• Communicate patient progress with supervisor and health team members
• Participate in staff training within the facility
• Record treatment charges per policy
• Instruct families or nursing staff in maintenance programs
