Job summary

Esperta Health seeks an experienced Wound Care Nurse (In-Home) to deliver expert wound care in patients’ homes. You will assess wounds, create care plans, and document thoroughly in electronic health records using WHG guidelines.

You will collaborate with an interdisciplinary team, educate patients and caregivers, and travel to client homes to provide evidence-based treatments while adhering to HIPAA regulations and safety standards.

Qualifications

• RN license active and unrestricted in the state of practice.
• BSN degree preferred.
• Minimum of three years wound care nursing experience preferred.
• WOCN or other wound care certification is a plus; exam eligibility within 60 days with reimbursement.
• Strong knowledge of evidence-based wound care guidelines and documentation in EHR.

Responsibilities

• Perform thorough wound assessments and physical examinations in patients' homes.
• Administer wound care treatments based on WHG and develop individualized care plans.
• Maintain accurate electronic health records in compliance with regulations.
• Educate patients and caregivers on wound care techniques and prevention.
• Perform wound care procedures within nursing scope or under supervision.
• Follow infection control protocols and ensure patient safety.
• Communicate effectively with patients and caregivers to address concerns.
• Maintain HIPAA privacy, security, and breach notification compliance.
