Job summary

Acts Retirement-Life Communities, Inc. is seeking a Wellness Home Services Licensed Practical Nurse (LPN) to provide personalized, one-on-one care for residents under RN supervision.

The role focuses on chronic conditions management, post-operative support, and addressing complex medical needs while ensuring safety and comfort. The ideal candidate holds an active LPN license, 1–2 years of clinical nursing experience, and CPR certification.

Qualifications

• Active LPN license in good standing with the applicable state Board of Nursing.
• Minimum of 1–2 years of clinical nursing experience, preferably in acute care or home health setting.
• Current CPR certification (or obtain within three months of employment).
• If transporting a resident, must maintain a valid state driver’s license and provide proof of current automobile liability insurance.

Responsibilities

• Deliver individualized, one-on-one care under the direction of a Registered Nurse (RN).
• Specializes in managing chronic conditions and supporting post-operative recovery.
• Performs medication management and vital sign monitoring.
• Collaborates with multidisciplinary care teams and educates residents and families.
