Job summary

Trinity Care Associates in Lowell, MA seeks an experienced case manager to deliver intensive, mobile case management and care coordination for consumers at the Respite site or in the community. You will support individuals with tailored interventions and connect them to essential services.

Responsibilities include developing individualized action plans, documenting progress clearly, facilitating groups, and advocating for entitlements across social, medical, psychiatric, and community resources

• Coordinate day-to-day needs and service delivery of consumers at Respite or in the community as assigned.

• Coordinate and facilitate services including income, insurance, entitlements, and access to community resources and medical/psychiatric scheduling.
• Develop IAPs (Individualized Action Plans) for each consumer, ensuring active participation in treatment planning.
• Maintain progress notes and documentation for IAPs and consumer contact, written timely and with clinical terms.
• Develop and facilitate groups for Respite consumers to support varied needs and community stability.
• Provide intensive supportive casework, advocacy and access to entitlements to social, medical, psychiatric and community resources.

Job description

• Provide intensive and mobile case management, care coordination and support and interventions to assigned consumers either in the community or at site based Respite.
• Coordinate and case manage the day to day needs and overall service delivery of consumers at Respite or in the community as assigned by Clinical Supervisor or Program Director.
• Support and assist consumers with day to day needs and overall service delivery providing direct support and community linkage to meet identified.
• Coordinate and facilitate services which may include assisting consumers with managing income, insurance, other entitlements, accessing other services and community resources and scheduling/coordination of all medical and psychiatric.
• Develop IAP’s (Individualized Action Plans) for each consumer as assigned. Ensure that each consumer is actively involved in their treatment planning process and their preferences are embraced.
• Maintain progress notes and all documentation related to IAP’s and consumer contact. Ensure that documentation is written in a timely manner and is measurable, observable, objective and utilizes clinical terms
• Develop and facilitate groups for consumers at the Respite program, as needed, that supports the varied needs of consumers (site and mobile) and support the skills necessary to maintain stability in the stability in the community.
• Provide intensive supportive casework for assigned caseload including advocacy and assistance with access to entitlements to social, medical, psychiatric and community.
