Job summary

Journey Found, Inc. is seeking an in-home support staff to help individuals live independently. You will provide daily living assistance, transportation, and skill-building activities in home or community settings.

The role requires a high school diploma, 3 years of driving, and certifications in PMT/CPR/FA with MAP completion. A flexible schedule and strong communication with guardians are essential to success.

Qualifications

• Must have at least a High School Diploma or equivalent.

• Excellent oral and written communication skills.

Responsibilities

Job Details: Job Location: 461 CG IHS - East Hartford, CT 06118, Salary Range: $20.85 Hourly, SCHEDULE: Mon & Tues 3p-7p, Thurs 12p-4p

GENERAL DESCRIPTION:

Provides people in their own home with support services designed to promote self-determination, independence, productivity, integration and inclusion in the community. May also provide job coaching services out of own or family home. As an IHS Support staff, you will be expected to work independently to assist people with any and all areas of their life in which they require assistance, maintain communication with family or guardian regarding support, and provide transportation as needed using your personal vehicle. You will be required to work a flexible schedule based on the needs of the person or people you are working with.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Activities of Daily Living May be required to assist the people we support with activities of daily living, which may include but is not limited to: personal hygiene/self-care tasks, dressing, cleaning, cooking, budgeting. Requirements will vary between programs based on the needs of the person supported.
• Provides therapeutic environment. Encourages participation; is helpful, supportive and respectful of differences with regards to unique needs, age, gender and culture of the person being supported.
• Documents goal progress daily.
• Health Maintenance. Assists people to participate in health related activities as required according to Journey Found and/or DDS policy, communicate all health needs with guardian.
• Implements skill building programs. Implements formal and informal skill building programs to help people learn new skills and increase independence.
• Manages behavioral crises. Implements formal Behavior Support Plan (BSP) with support from the clinical team, where applicable. Applies crisis prevention and intervention strategies, as approved by Journey Found.
• Effective Interaction. Manages individual people in their home and in the community. Works effectively with one or more people, encouraging participation in all programs and activities, providing support and offering assistance.
• Transportation. Provides transportation for people supported according to Journey Found transportation policies.
• Insurance Must maintain minimum insurance coverage and be able to provide documentation of such coverage upon request.
• Community relations. Promotes a favorable image of the people supported and the organization by fostering good community relations by demonstrating appropriate professional behavior.
• Must be able to work independently and work closely with guardian/family members.
• Documents and notifies management of any issues brought forth that effect the therapeutic care or environment.

Requirements:

• Must have at least a High School Diploma or its equivalent
• Must have at least 3 years driving experience with a valid driver’s license
• Behavioral Management Experience and/or formal training
• Ability to communicate, both orally and in writing
• Will be required to be PMT, CPR, FA and complete DDS Medication Administration Program (MAP)
• Must be free from communicable disease
