Job summary

Choice Home Health in San Luis Obispo County is seeking a licensed Speech-Language Pathologist to provide in-home therapy services to patients with speech, language, cognitive-communication, and swallowing disorders.

We offer full-time and per diem SLP positions with competitive pay of $60.00-$70.00 per hour, plus benefits such as PTO, health/dental/vision, mileage reimbursement, and continuing education support. Experience in home health and HCHB documentation is preferred.

Qualifications

• Master's degree in Speech-Language Pathology and active CCC license.
• Minimum of one year as a Speech-Language Pathologist; CFYs not accepted.
• Prior home health experience and HCHB documentation experience preferred.

Responsibilities

• Provide in-home speech therapy services for speech, language, cognitive-communication and swallowing disorders.
• Complete initial evaluations and develop individualized plans of care.
• Deliver skilled therapy visits and reassess patient progress regularly.
• Document patient progress and communicate changes to the care team and physician.
• Collaborate with an interdisciplinary team to coordinate patient care.
• Educate patients and caregivers to support independence and successful outcomes.
• Complete required documentation, including OASIS when applicable.
