Job summary

Frontier Home Health in Pueblo, Colorado is seeking a licensed Speech-Language Pathologist to evaluate, plan, and administer home-based speech-language therapy for patients in their residences.

Qualifications

• Current, unrestricted state license as an Speech-Language Pathologist.
• Graduate of an accredited Speech-Language Pathology program.
• Minimum two years of clinical experience in acute care or rehab; home health preferred.
• Knowledge of Medicare CoPs and home health regulations.
• Valid driver’s license and reliable transportation.
• Current CPR certification.

Responsibilities

• Performs comprehensive speech-language evaluations and home-based assessments.
• Develops individualized treatment plans with measurable goals.
• Implements dysphagia, cognitive-communication, voice, and speech therapies.
• Coordinates care with physicians, nurses, social workers, and caregivers.
• Documents findings and progress within required timeframes.
