Job summary

Qualifications

• Three years' experience in a social agency, hospital, or health/welfare organization preferred.

Responsibilities

• Makes the initial patient assessment as ordered.
• Documents assessments, visits, and contacts on the patient's chart.
• Places phone calls and makes visits, serving as a resource liaison as appropriate.
• Provides patients and families with community resources.
• Attends weekly team conferences.
• Performs other job-related duties as assigned.
