Responsibilities

• Provides skilled nursing care in the home to patients per the service plan and physician orders
• Performs nursing care ensuring patient safety and reducing medical errors
• Educates patients and families on health maintenance and self-care
• Manages clinical notes and visit verifications in a timely manner
• Maintains confidentiality and adheres to infection control policies
• Participates in on-call schedule and communicates schedule changes promptly
