Job summary

Lykos Mediacl, part of Brigade Health and its Affiliates, seeks a Physician Assistant or Nurse Practitioner to deliver continuum-based care in home and community settings for older adults. You’ll work with a multidisciplinary team to provide high-quality, relationship-based care across geriatrics, psych, and neurology.

A supportive, collaborative culture fosters growth. Active NP/PA license in the practicing state and 1–2 years in geriatrics or primary care are preferred.

Qualifications

• Active NP or PA license in the practicing state.
• 1–2 years of clinical experience in geriatrics or primary care preferred.
• Compassionate, patient-centered approach and strong clinical judgement.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver personalized high-quality care to seniors in homes, assisted living, hospice, and nursing facilities.
• Provide dementia-informed, relationship-based care across the continuum of care.
• Conduct comprehensive assessments and develop tailored care plans for older adults.
• Manage chronic conditions with empathy for patients and families.
• Collaborate with cross-functional partners (palliative, psych, neuro) for coordinated, whole-person care.
• Build trusted relationships with residents, caregivers, and facility partners.
• Maintain consistent documentation, follow-through, and patient communication.
