Job summary

WellBe Senior Medical is seeking a part-time Registered Nurse (RN) to provide in-home care across the Struthers, Ohio area. The role includes urgent/episodic visits, telehealth consults with APPs or physicians, and care coordination with an interdisciplinary team.

You will perform comprehensive assessments, wound care, medications administration as ordered, and patient education while maintaining complete documentation and compliance with state and federal regulations.

Qualifications

• Active State Licensure required; compact licensure preferred.
• 3+ years as an RN in direct, patient-facing care delivery role (required).
• CPR Certification is required.
• Prior Telehealth experience (preferred).
• Home care experience (preferred).
• Experience in hospice, palliative care, or geriatrics strongly preferred; other complex patient health care experience will be considered.

Responsibilities

• Deliver urgent, in-home patient care under the direction of a licensed provider throughout the assigned market.
• Perform comprehensive home safety evaluations and document findings.
• Obtain patient histories, review medications and treatment plans, and perform physical assessments.
• Document patient care and ensure timely update of records at each encounter.
• Collaborate with APPs, nurses, social workers, and interdisciplinary team to plan care.
