Job summary

Home Health of Western Colorado in Grand Junction is seeking a Registered Nurse to provide skilled, compassionate in-home care across Western Colorado. • Licensed RN eligible to practice in Colorado.
• Skilled in comprehensive patient assessments and care planning.
• Experience with wound care and post-surgical care is preferred.

Responsibilities

• Perform comprehensive in-home patient assessments and develop care plans.
• Administer medications and treatments as prescribed; monitor client responses.
• Provide wound care and post-surgical support as needed.
• Coordinate with interdisciplinary team and educate patients and families.
• Document care accurately and triage patient needs to ensure safe transitions of care.
