Qualifications

Current Registered Nurse (RN) License for the state in which the nurse practices.
Current Health Certificate (per facility RN contract or state regulation).
Current PPD or Chest X-Ray.
Current BLS card.
One year prior RN experience preferred.

Responsibilities

Utilizes the nursing process to plan, implement, and evaluate patient care for clients in home health settings.
Initiates the plan of care and updates it as patient needs change.
Incorporates therapeutic, preventive, and rehabilitative nursing actions based on diagnosis and goals.
Intervenes in response to changes in patient condition and reports to physician or supervisor.
Administers medications and treatments, monitoring effectiveness and documenting results.
Reviews plan of care and coordinates education, discharge planning and care coordination as applicable.
