Job summary

Allina Home Health in Minneapolis seeks a registered nurse to provide in-home care to patients and families, coordinating and evaluating care plans with leadership and other disciplines. This role emphasizes therapeutic nurse-patient relationships and safety in care delivery.

The position requires RN licensure in MN, 0–2 years of RN experience, and a BSN is preferred. Day shifts, 1.0 FTE, and every 4th weekend rotation are included with comprehensive benefits.

Qualifications

• Required: Associate's or Vocational degree in Nursing; 0–2 years RN experience.
• Preferred: Bachelor's degree in Nursing; 0–2 years homecare/hospice experience.

Responsibilities

• Comprehensive assessment of patient and family needs; establish plan of care and coordinate with family and patient.
• Maintain education and competencies to provide high-quality patient care.
• Incorporate evidence-based nursing practice respecting current statutes and regulations.
• Coordinate and delegate to ensure safe, effective care.
• Other duties as assigned.
