Job summary

Prestige Home Health Care LLC in Phoenix, AZ is seeking a compassionate Registered Nurse (RN) to join our home health team. You will conduct patient assessments, develop care plans, and deliver skilled nursing services in patients' homes.

The ideal candidate will hold an active AZ RN license with BLS/CPR, at least 1 year of clinical nursing experience (home health preferred), and strong wound care/IV therapy knowledge. A valid driver’s license is required for home visits.

Qualifications

• Active Arizona RN license in good standing.
• Current BLS/CPR certification required.
• Minimum 1 year of clinical nursing experience; home health experience preferred.
• Strong knowledge of skilled nursing procedures, wound care, and IV therapy.
• Excellent communication, time management, and organizational skills.
• Ability to work independently and manage a patient caseload efficiently.
• Valid driver's license and reliable transportation for home visits throughout Phoenix, AZ.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications, treatments, and skilled nursing procedures in a home setting.
• Monitor and document patient progress, vital signs, and changes in condition.
• Coordinate care with physicians, therapists, and other healthcare team members.
• Educate patients and family members on disease management, medications, and home care techniques.
• Ensure compliance with state, federal, and agency regulations and standards of care.
• Maintain accurate and timely clinical documentation using electronic health records (EHR).
