Elliot Health Syste is hiring an RN Case Manager for weekend shifts with holiday rotation and on-call backup. The role supports in-home care for patients in Manchester and Southern New Hampshire through the VNA program.

The RN will deliver care across the nursing process, coordinate with families, and uphold professional standards within an interprofessional team. This full-time position includes a $10,000 bonus eligibility and comprehensive benefits.

Qualifications

• Experience as an RN delivering care using the nursing process.
• Background in home health or case management is preferred.
• Ability to participate in weekend and holiday rotations.
• Familiarity with interprofessional practice standards and ethics.
