Job summary

Pathways Healthcare is seeking a dedicated Registered Nurse to provide per diem home health care from our Concord, MA office, serving Lowell, Tyngsborough and Groton.

Responsibilities include initiating preventive and rehabilitative nursing procedures, administering medications, conducting initial patient assessments, and collaborating on robust Plans of Care with physicians and the care team. MA RN license and BLS CPR are required, plus a valid driver's license and vehicle.

Qualifications

• Active RN license in MA.
• Preferably 2+ years nursing experience in geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and strong communication.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Valid driver's license and own a reliable insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
