Job summary

GrandCare Health Services is seeking a Physical Therapist Assistant (PTA) to deliver in-home physical therapy to patients in the San Fernando Valley area. You will work under the supervision of a licensed Physical Therapist, focusing on orthopedic rehab, post-surgical recovery, and patient-centered outcomes.

The role offers autonomy in the field with strong clinical support, flexible scheduling, and opportunities for growth within Southern California’s leading home health provider.

Qualifications

• Valid California PTA license.
• Current CPR Certification.
• 1+ year PTA experience; home health experience preferred.

Responsibilities

• Deliver hands-on in-home physical therapy to restore function and mobility.
• Assist patients with therapeutic exercises, gait training, transfers and prosthetic training.
• Apply clinical physical agents such as heat, cold, ultrasound, and massage.
• Educate patients on assistive devices and safe home exercise.
• Complete accurate and timely clinical documentation.
• Communicate patient progress and changes to the supervising Physical Therapist.
