Job summary

At Home Healthcare of Middle Tennessee seeks a skilled home health nurse (LPN or RN) to deliver in-home care, follow established care plans, and promote patient safety and independence in Kentucky.

You will work under the Director of Patient Care Services, maintain accurate notes, perform patient assessments, and participate in on-call coverage. A valid Kentucky nursing license and CPR are required.

Qualifications

• Valid Kentucky nursing license (LPN or RN).
• Current CPR certification or willingness to obtain.
• Current TB skin test or willingness to obtain.
• Competency in general nursing skills.

Responsibilities

• Provides skilled nursing care in the home per the service plan and physician orders.
• Maintains patient safety and reduces medical errors.
• Promotes patient health through teaching and rehab.
• Assists patients in learning self-care techniques.
• Prepares progress notes and follows the client service plan.
• Submits notes and visit verifications timely.
• Keeps abreast of nursing trends and knowledge.
• Communicates patient health status to staff.
