HarmonyCares is seeking a Nurse Practitioner to deliver high-acuity, in-home primary care in Michigan. Working within an integrated, team-based model, you will partner with physicians, nurses and care coordinators to provide patient-centered care in the home setting.

This role focuses on care over volume, with responsibilities including comprehensive assessments, care planning and coordination, education for patients and families, and adherence to value-based care standards.

Qualifikationen

• Licensed NP in the state where HarmonyCares Medical Group office is located.
• Federal DEA license or eligibility required.
• Geriatrics training or experience preferred.
• Active CPR/BLS certification.
• Board certification in advanced practice nursing.
• Valid driver's license with a good driving record.

• Provide direct home-based primary care to high-acuity patients.
• Develop and implement patient-centered care plans.
• Collaborate with physicians, nurses and care coordinators.
• Educate patients and families and coordinate follow‑up.
