Job summary

Preferred Home Health Care & Nursing Services is seeking a dedicated Physical Therapist Assistant to deliver in-home and community-based rehabilitation under the supervision of a licensed Physical Therapist. You will implement therapeutic exercises, gait and balance training, and functional mobility activities to improve safety and independence for seniors and individuals with disabilities.

You will monitor client progress, document visits and outcomes, and communicate changes with the care

Qualifications

• PTA training/credential required.
• Strong patient rapport, empathy, and ability to work with diverse populations.
• Knowledge of basic PT exercises and mobility training techniques.
• Attention to detail for documentation and adherence to care plans.
• Effective communication and teamwork skills.

Responsibilities

• Administer in-home therapeutic exercises and mobility training per the care plan.
• Support gait, balance, and functional movement to reduce fall risk and promote independence.
• Monitor client progress and observe responses to interventions.
• Document therapy visits and outcomes accurately.
• Communicate progress and notable changes to the care team.
