Job summary

CenterWell Home Health seeks a licensed Physical Therapist to provide home-based care, helping patients regain strength, mobility, and independence in their own homes. You will work under the Clinical Manager to deliver functional improvements and coordinate with a multidisciplinary team.

Responsibilities include executing treatment plans, documenting records weekly, and participating in department in-services. Requires state PT license, valid driver’s license, CPR, and English fluency.

Qualifications

• Licensed Physical Therapist in the state
• Valid driver’s license, auto insurance and reliable transportation
• Current CPR certification
• Experience collaborating with a healthcare team
• Knowledge of geriatric physical, emotional, social, and biological changes
• Read, write, and speak fluent English

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes, as developed by the Physical Therapist
• Perform non-complex manual therapies and modalities (exercises, massage, traction, heat/cold, electrical modalities)
• Document client records and submit weekly to the office
• Attend educational meetings and coordinate department in‑services
• Notify Physical Therapist or Branch Director of equipment failures, supplies, or staff or client changes
• Apply infection control and universal precautions in patient care
