Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care in patients' homes or senior living communities in Washington Court House, OH. You’ll evaluate, plan, and provide evidence-based therapy, documenting in FOX’s EMR and collaborating with an interdisciplinary team to promote independence and aging in place.

Qualifications

• Basic computer and EMR proficiency.
• Current CPR certification.
• Valid PT license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.
