Job summary

Qualifications

• Licensed PT in good standing with state licensing board.
• Current CPR certification as required by state regulations.
• Reliable transportation for home visits.

Responsibilities

• Assess muscle strength, mobility, gait, ROM, and rehab potential; help develop the care plan.
• Direct and adjust the PT treatment plan and goals with patient and team.
• Maintain accurate clinical notes and document progress weekly per guidelines.
