CommonSpirit Health at Home is seeking a dedicated Physical Therapist to provide in-home rehabilitation services in the Aurora, South, and Southeast Denver areas. You will assess patients, create personalized treatment plans, and collaborate with families and healthcare teams to promote recovery and quality of life.

The role involves hands-on therapy, patient and caregiver education, and documentation, with benefits including health coverage and a retirement plan.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Active PT license in the practicing state.
• Current CPR/BLS certification.
• Driver’s license required.

Responsabilités

• Evaluate patients’ physical abilities, mobility, strength, and functional limitations.
• Administer therapeutic exercises, manual therapy, and modalities to improve function.
• Educate patients and families on exercises, safety, and lifestyle modifications.
• Monitor progress and adjust treatment plans to achieve optimal outcomes.
• Maintain accurate patient records in compliance with regulations and policies.
