Job summary

Excellent Home Care NY is seeking a licensed Physical Therapist (PT) to join our compassionate home care team serving New York City and surrounding areas. You will evaluate patients, develop treatment plans, educate families, and coordinate with the interdisciplinary team to restore function and promote independence in the home setting.

Ideal candidates have NY licensure, at least one year in acute care, and strong assessment skills.

Qualifications

• Has graduated from a Physical Therapy curriculum approved by the APTA or AMA Council.
• A minimum of one year of experience as a PT in an Acute Care setting; two years preferred; home care desirable.
• Current NY State Physical Therapist license/registration.

Responsibilities

• Planning, provision and coordination of Physical Therapy to agency patients.
• Evaluate patient functional level and train families; provide assistive devices as needed.
• Collaborate with health team to set rehab and maintenance goals.
• Evaluate needs and deliver written progress reports; adjust plans accordingly.
• Identify equipment safety issues and ensure follow-up with vendors or agency.
