Black Women's Mental Health Institute is seeking a Physical Therapist to provide in-home physical therapy to patients while managing an assigned caseload. The role requires conducting patient assessments and developing individualized care plans in collaboration with physicians.

You will deliver prescribed therapies, document progress meticulously, and coordinate with multidisciplinary teams to achieve optimal outcomes.

Qualifications

• Licensed/credentialed physical therapist with strong clinical assessment skills.
• Ability to create and implement individualized treatment plans.
• Proficiency in patient documentation and clinical reporting.
• Excellent communication and collaboration with physicians and care teams.
• Experience or aptitude for supervising therapy assistants.
