Job summary

Allina Health in Minneapolis, MN is seeking a licensed Physical Therapist for a full-time Day shift. You will provide physical therapy and education to clients and may case manage clients in the assigned geographic region.

Responsibilities include initiating assessments, collaborating with clients and families, coordinating referrals, and maintaining plan of care while monitoring progress. A Bachelor’s degree in Physical Therapy and MN licensure are required; 2–5 years in rehab or long-term care

Qualifications

• Bachelor's degree in Physical Therapy from an APTA accredited program.
• 2 to 5 years in a rehabilitation or long term care setting.
• Licensed Physical Therapist in MN State Board required upon hire; MN BLS certification preferred or in-house training.

Responsibilities

• Provides Physical Therapy treatment to clients.
• Initiates assessment of client per diagnosis.
• Consults with client on goals and outcomes.
• Educates client and family on treatment plan and cultural considerations.
• Produces ongoing plan of care and initiates referrals to other disciplines.
• Case management and coordination of care across geographic regions.
