Job summary

Genesis HealthCare is seeking a Physical Therapist for in-home outpatient rehabilitation in the Nashua area, including NH communities. The role is part-time (18-29 hours/week) with responsibilities across evaluation, treatment, and caregiver education in patients’ homes and community settings.

The position requires a valid PT license in the relevant state, and a driver’s license for on-site home visits. Experience in home health is a plus; opportunities exist for mentorship and ongoing CEU

Qualifications

• Must hold a valid Physical Therapy license in the state(s) of practice.
• Must have the formal education as described (PT degree with options shown).
• A valid driver's license or reliable transport is required for home visits.

Responsibilities

• Provide outpatient rehabilitation in in-home settings for older adults.
• Develop and implement individualized PT plans of care.
• Coordinate with rehab team and refer to other disciplines as needed.
• Educate patients and caregivers to promote independence and safe mobility.
• Maintain documentation and comply with licensure requirements.
