Job summary

CenterWell Home Health is seeking a licensed physical therapist to provide in-home care to patients in the East Central Tucson area. You will plan, implement, and document PT treatments in patients’ homes, collaborating with a multidisciplinary team to achieve functional goals and improved independence.

The role requires an unrestricted PT license, CPR certification, and ability to cover within the East Central Tucson area. A $10,000 sign-on bonus is available for this full-time position.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Must be able to provide coverage within the East Central Tucson area.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to improve function.
• Assess patients' physical strengths to guide treatment goals.
• Develop rehabilitation programs aligned with physician plans and patient goals.
• Instruct PTAs and Home Health Aides with care instructions and supervise their documentation.
• Document observations, interventions and evaluations promptly.
• Report progress to physician and care team and submit required evaluations, plans and discharge summaries.
• Contribute to patient impact through skilled therapy.
