Job summary

FOX Rehabilitation is hiring licensed Physical Therapists to provide in-home and senior living care in West Covina, CA, including telehealth options. You will manage your own treatment schedule, build meaningful patient relationships, and help older adults improve mobility and independence.

Qualifications

• Valid Physical Therapy license in the state of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Current CPR certification and basic computer/EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility and functional independence.
• Document in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with PTs, OTs, SLPs and Fitness Specialists.
