Powerback Rehabilitation To You is seeking a licensed Physical Therapist to provide outpatient rehabilitation in home and independent living settings across the nation. You will develop personalized therapy plans, educate patients and caregivers, and collaborate with a multidisciplinary team to improve function and quality of life.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Master's degree in Physical Therapy or equivalent required; additional PT related degree accepted.
• Bachelor's degree in Physical Therapy required; 5 years of progressive PT experience beneficial.

Responsibilities

• Provide outpatient rehabilitation services in home and independent living settings.
• Develop and deliver individualized physical therapy plans to restore, reinforce, and enhance function.
• Collaborate with rehab team to address patient needs and coordinate care.
• Supervise PT Assistants and ensure compliance with licensure requirements.
• Maintain timely documentation and ensure adherence to schedules.
